ESI computerization and submission of declaration in form1 for smart card

As a part of computerization, modernization& automation of ESI Corporation has launched a project called “Project panchadeep” to increase the performance of the scheme and faster benefits to the insured employees. The computerization project is undertaken by WIPRO Technologies. For this purpose, the employer code has been converted into 17 digits. This code will be available at your branch office or at the regional office. The detail of the code is as shown below.

A fresh declaration (from1) has to be submitted for the purposes of issuance of Smart card.

All employees and his or her family (if required) will be given a smart card instead of traditional paper card as a part of computerization. Smart card is a plastic card just like ATM Card. The smart card will be issued with updated details of the employee and his family members. For the issuance of smart card an up to date database is created, for this purpose all employers are required to get filled a fresh declaration form in form 1 from employees and handed over to ESIC branch office.

After entering all data into the system, an enrollment camp will be organized for taking family photograph and getting bio-metric details. The smart card will be delivered to employees on to his doorstep. Two cards will be issued one for insured person and other one for his family members who is eligible under the scheme.

Important Points to note

• Declaration form must be submitted in A4 size paper only any other size will not accept by esic.
• Form must be submitted without overwriting
• All columns must be filled correctly.

Benefits of computerization of ESIC

1. The employees shall be able to receive medical benefits from any dispensary and cash benefits from any Branch office.
2. The employer can pay the contribution online.
3. Online registration of employers and employees are possible.

Project panchadeep is expected to be rolled out on 10 th aug 2010. .The success of this big projects requires the co-operation from the insured and employer. Let us make this project success as the ultimate benefit is for us.  Furthermore, if we are not submitting the declaration form now, the beneficiary will have to face difficulties in getting smart card later.
