ESI computerization and submission of declaration in form1 for smart card
As a part of computerization, modernization& automation of ESI Corporation has launched a project called
“Project panchadeep” to increase the performance of the scheme and faster benefits to the insured employees. The computerization project is undertaken by WIPRO Technologies. For this purpose the employer code has been converted in to 17 digits. This code will be available at your branch office or at regional office. The details of the code is as shown below
A fresh declaration (from1) has to be submitted for the purposes of issuance of Smart card.
All employees and his /her family (if required) will be given a smart card instead of traditional paper card as a part of computerization. Smart card is a plastic card just like ATM Card. The smart card will be issued with updated details of the employee and his family members. For the issuance of smart card an up to date database is created, for this purpose all employers are required to get filled a fresh declaration form in form 1from employees and handed over to ESIC branch office.
After entering all data in to system, an enrollment camp will be organized for taking family photograph and getting bio-metric details. The smart card will be delivered to employees on to his doorstep. Two cards will be issued one for insured person and other one for his family members who is eligible under the scheme

Important Points to note
- Declaration form must be submitted in A4 size paper only any other size will not accept by esic.
- Form must be submitted without overwriting
- All columns must be filled correctly.
Benefits of computerization of ESIC
- The employees shall be able to receive medical benefits from any dispensary and cash benefits from any Branch office.
- The employer can pay the contribution online.
- Online registration of employers and employees are possible.
Project panchadeep is expected to rolled out on 10 th aug 2010 .The success of this big projects require the co-operation from the insured and employer. Let us make this project success as ultimate benefit is for us. Also if we are not submitting declaration form now, beneficiary will have to face difficulties in getting smart card later.

1. get a new 17 digit employee code from your branch or regional office.
2.Employer has to submit a fresh Form 1, to your branch office after get it filled from employees.
3.Presently employee can use paper card. but after the completion of computerization , employee must use new smart card.

Dear Sir,
I am working for one of Delhi based IT company for Nagpur location from last 2 years.
As said above , my employer is at Delhi & he has issued me card which
is acceptable at Delhi only.
Also while filling card he has corrected my residential address as
Delhi & submitted to you.
Now I have no time to fill 3 month service form to take ESIC service in Nagpur. Also its not so easy to take employer sign every time on that form after every 3 months.
My regular contribution to ESIC is from my joining Rs.700/-pm(2yrs).
Is there any rule to punish employer for this ?
What can ESIC do against employer for this.
I want card only acceptable in Nagpur.
Kindly give me suggestion.

Hello sir,
I am working in an private company. Here we are more than 150 employees covered under ESI. In this, around 20 of them have ESI permanent Card. Also they are working in other state and their family staying in other state. So, the PIC which they already have is enough for getting treatment now. They are saying that without family members how should i take photo for smart card.
Please give solution like this cases.
when can i use esi card or in how many places in india i can use this card? can avail the same facility after leaving the current employer?
