i have original smart card but no use. some hospital no computerized smart card doctor ask i want temprory card please all hospitals make computerized

I am Alpana Gaikar.
My dad was working in Mahalaxmi Glass Co.company had closed.he lost his ESI card.Is there there any way to get duplicate card ?Can he enjoy medical facility by paying money to Kamgar hospital.
